Intake-Exit

Used to record and track additional client profile data related to Intake and Exit. This area is a 1:1 relationship to the client profile meaning there will only ever be one Intake/Exit record per Client Profile.

Details

  • How Heard:  Determines how the client heard about the program.  List pulls from Marketing Sources select list.  This field differs from the "How Heard" field in Assignments where "How Heard" refers to each individual Assignment versus the overall enrollment as it does here.
  • Referred By:  Referral source pulled from the user-defined Company list.
  • Previous Year: Was the client enrolled with your organization last year?  Yes/No checkbox
  • History:  Text field listing program years client was enrolled in programs such as 03-04, 04-05, 05-06. 
  • Termination Date: Date of final termination when the client will no longer be enrolled in ANY service.  This date may or may not be the same as an end date for a specific service.
  • Re-Entry Date: If the client was terminated and returned in the same program year, this is the date of return.  Clear out the termination information and the client remains enrolled.  You may desire to make a notation in the Client Case Notes of these details.  If the return is in a different year, the client may be considered reenrolled based on your organization's policies.
  • Carry Over: Should the client be carried over into the next program year?  Yes/No checkbox. This is one of several options for carrying data forward into the next program year for organizations that have each year separated into different databases.
